Medicare and Medicaid in Florida: Understanding Dual Eligibility and D-SNP Plans
August 21, 2026Choosing how to receive Medicare coverage is one of the most important decisions you will make when becoming eligible. Two of the best-known options are Medicare Advantage and Original Medicare paired with Medicare Supplement Insurance, also called Medigap.
These options are not interchangeable. They organize benefits differently, calculate costs differently, and can provide different levels of provider flexibility. You cannot use a Medicare Supplement policy to pay Medicare Advantage copayments, deductibles, or premiums.
The better choice depends on your healthcare needs, preferred doctors, prescriptions, budget, travel habits, and comfort with networks and out-of-pocket expenses.
Start With Original Medicare
Original Medicare includes Part A and Part B.
Part A generally helps cover inpatient hospital care, limited skilled nursing facility care, hospice, and certain home health services. Part B generally helps cover physician care, outpatient services, preventive care, durable medical equipment, and other medically necessary services.
Original Medicare does not cover every healthcare expense. Beneficiaries can remain with Original Medicare and add separate coverage, or they may receive Medicare benefits through a Medicare Advantage plan offered by a Medicare-approved private insurance company.

What Is Medicare Advantage?
Medicare Advantage is also known as Medicare Part C. It is another way to receive Medicare Part A and Part B benefits through a private insurer approved by Medicare.
You must continue paying the Medicare Part B premium. A Medicare Advantage plan may charge an additional plan premium, although costs vary by plan and service area.
Most Medicare Advantage plans include Medicare Part D prescription drug coverage. Plans may also offer benefits Original Medicare does not generally cover, such as certain dental, vision, hearing, fitness, transportation, or over-the-counter benefits.
Additional benefits are plan-specific. Availability, limits, provider participation, and costs must be reviewed for the exact plan offered in your ZIP code.
What Is Medicare Supplement Insurance?
Medicare Supplement Insurance, or Medigap, works alongside Original Medicare. It helps pay certain out-of-pocket expenses left by Parts A and B, such as deductibles, copayments, or coinsurance, depending on the standardized plan selected.
Medigap policies are identified by letters. In most states, policies with the same letter provide the same basic benefits, although premiums and insurance companies can differ.
A Medicare Supplement policy does not replace Original Medicare. Medicare remains the primary coverage and the supplement helps with eligible remaining costs.
Medigap policies sold today generally do not include outpatient prescription drug coverage. Someone using Original Medicare and Medigap will usually need to consider a separate Part D plan.
You Cannot Have Medicare Advantage and Medigap Together
A Medicare Advantage plan and a Medicare Supplement policy serve different purposes. You cannot use both at the same time.
If you join Medicare Advantage, you continue to have Medicare but receive Part A and Part B benefits through the private plan. A Medigap policy cannot pay the Medicare Advantage plan’s out-of-pocket costs.
If you want to move from Medicare Advantage to Original Medicare and Medigap, enrollment timing and Medigap eligibility can matter. Do not cancel existing coverage until you have confirmed when the new coverage can begin and whether the Medigap application has been approved.
Provider Access and Networks
Medicare Advantage
Many Medicare Advantage plans use provider networks. Depending on whether the plan is an HMO, PPO, or another type, you may need to use network doctors and hospitals to receive the lowest costs. Some plans require referrals or prior authorization for certain services.
Before enrolling, confirm that your primary doctor, specialists, preferred hospitals, pharmacies, and other providers participate in the plan’s current network.
Original Medicare With Medigap
Original Medicare generally allows you to visit any doctor or hospital in the United States that accepts Medicare. Because Medigap works with Original Medicare, it can offer broader nationwide provider flexibility.
This may be important to Florida residents who travel frequently, divide time between states, or want access to specialists outside a local network.
How Costs Are Structured
Medicare Advantage Costs
Medicare Advantage plans frequently use copayments, coinsurance, deductibles, and an annual maximum out-of-pocket limit for covered Part A and Part B services. The exact amounts vary by plan.
Some plans have low or $0 additional monthly premiums, but a $0 plan premium does not mean healthcare is free. You must continue paying the Part B premium and may owe costs as you receive services.
Medigap Costs
With Medigap, you pay a separate monthly premium in addition to the Medicare Part B premium and any Part D premium. In exchange, the selected policy may make eligible out-of-pocket costs more predictable.
Premiums can vary based on the insurer, location, age, rating method, available discounts, and other permitted factors. Premiums may increase over time.
Prescription Drug Coverage
Most Medicare Advantage plans include Part D prescription drug coverage. Review the plan’s formulary to ensure your medications are covered and determine the applicable tiers, copayments, pharmacy network, restrictions, and prior authorization requirements.
Medigap policies do not generally include current Part D coverage. Beneficiaries using Original Medicare and Medigap commonly enroll in a separate prescription drug plan.
A person can face a Part D late enrollment penalty after going 63 days or more without Part D or other creditable drug coverage, unless an exception applies.
Dental, Vision, and Hearing Benefits
Original Medicare does not generally cover routine dental care, standard eye exams for glasses, hearing aids, or routine hearing exams.
Some Medicare Advantage plans include benefits for these services. Coverage varies, and benefits may include provider networks, annual allowances, service limits, or cost sharing.
Medigap focuses on expenses associated with Original Medicare. Separate dental, vision, or hearing coverage may be needed.
Prior Authorization and Referrals
Original Medicare generally does not require prior authorization for most covered services, although some exceptions apply.
Medicare Advantage plans may require prior authorization for specified services, treatments, supplies, or medications. Some plans may also require referrals before seeing specialists.
Review the plan’s rules if you receive ongoing specialty care, scheduled treatments, therapy, durable medical equipment, or expensive medications.
Travel and Living in More Than One State
Florida has many seasonal residents who spend part of the year in another state. Provider access away from home can therefore be an important consideration.
Original Medicare with Medigap may provide broader nationwide flexibility when providers accept Medicare.
Medicare Advantage plans must cover emergency and urgently needed care according to Medicare rules, but routine nonemergency care outside the service area may be limited or cost more. PPO plans may offer some out-of-network coverage, while HMO rules are usually more restrictive.
Ask how the exact plan handles routine care, prescriptions, follow-up visits, and extended travel.
Medigap Open Enrollment
Federal law provides a six-month Medigap Open Enrollment Period. It begins the first month you have Medicare Part B and are age 65 or older.
During this period, you generally have stronger protections when purchasing a Medigap policy. After the period ends, an insurer may be permitted to use medical underwriting unless you have a guaranteed issue right or another protection applies.
This makes the initial choice important. Someone who selects Medicare Advantage may not always be able to purchase a preferred Medigap policy later without answering health questions.

Medicare Advantage Enrollment Periods
Medicare Advantage enrollment and plan changes are limited to specific periods, including the Initial Enrollment Period, the annual Medicare Open Enrollment Period from October 15 through December 7, the Medicare Advantage Open Enrollment Period from January 1 through March 31 for current Medicare Advantage members, and qualifying Special Enrollment Periods.
Not every enrollment period allows every type of change. Confirm your eligibility before attempting to leave or change coverage.
